Davroar the spider moves into the chamber on the ceiling. If there was any danger in the cavern, he didn't see it. He moves quietly now, hoping that the noise he made earlier didn't alert creatures. He stops over the doors that are opposite the corridor that the others are waiting in. Since he can see in the dark in this form, he notes that the doors are likely an entrance to a squarish room that seems to have been built inside the natural caverns. Ahead of him (to the south) is a set of steps leading down into unexplored areas filled with more rubble.
Another poor roll for you pal, this time on perception. I'll reveal more of the map when others enter with light. If there's danger in this chamber, it has not revealed itself yet.
DM Kriv enters the cavern and sheds light. Ahead of him is a large pile of rubble. His eyes focus on it and he sees movement but he can't make out what it is and he's not really sure of what he saw. Please move token.
He moves up with Kriv and the light, to stand underneath Davroar and peer around in the gloom.
South would seem to lead back the way they came, roughly, so he's more in favour of a northerly route. Still, it doesn't hurt to have a quick look around!
Dugan, Davroar took his action. All that means is that we have a couple of players that haven't posted yet.

DM Kriv's warning, perhaps unheeded or too late, did not prevent anyone from marching into the cavern. And yes indeed, there was some movement, and it was near where Bern had ventured. Suddenly it was upon the ranger. A large (roughly 8 feet long) darkly colored worm or snake-like creature emerges from the rubble. It lashes out with four barbed tentacles which ring its beak and attacks Bern. Fortunately, even though he didn't expect it, it fails to hit him.
Another one crawls out of the rubble and strikes out at Ryssa and she is not so lucky. It hits her with the tentacles and draws her into it's maw and bites her. The same fate befell Chandan. Not even Davroar could avoid battle. These critters crawl on the wall. The monstrosity grabs him but the spider eludes the bite.
Ryssa takes 8 points of damage. Chandan took 12 points and Davroar 5

Kriv will be the only one to act this round, the surprise round, as the collective stealth rolls of this(these) foes, the ones within the line of sight, exceeded the passive perceptions of the party. These critters have advantage on stealth rolls. I allowed extra rolls for active perception, which Kriv made. Kriv gets his attack and then everyone, roll for initiative.
The Dragonborn noble shared a frustrated, despairing glance with Luminmorn, a single unspoken word encompassing his exasperation with the sophonts that blithely stumbled into this worm-nest.
Mammals.
A brief, approving nod towards Jaliera for being the one unscaled member of their group not currently imperilled, and he strode forth to assail the one currently trying to devour Ryssa, bringing his maul crashing down.

These tentacles are all over. She sees Kriv going to help Ryssa and so she uses her bow to try to help Davroar as he is on his own whilst on the ceiling.
Jaliera Initiative Gypsy rolled 1d20+3 and got 20
As her initiative is high, I rolled her attack. Apologies for the numerous to hit rolls, used the firt, then rolled damage & hide. Not sure if sneak damage applies. Thanks!
The cleric screamed when the tentacled beast drew her into its maw and bit her. Not trusting herself to have sufficient concentration to cast a spell, she drew a dagger and attacked.
